Terms and conditions for submitting the form

- I understand that this form does not exempt me from the requirements of the course and that accommodations made for assignments remain at the discretion of each instructor, in accordance with the regulations in effect in the faculty or unit responsible for each of my courses.

- I understand that adequate participation in the learning activities of the Pharm. D. program will be taken into consideration when evaluating professionalism. Irregular attendance or lack of effort may result in dismissal from the program.

- I understand that I may submit this form without supporting documents. However, if deemed necessary, the unit may contact me for additional information regarding the request and or require supporting documents.

-I understand that if the absence request is not submitted within a reasonable period of time, the unit may contact me to request more information about the delay in submitting the request.
